We are thrilled to welcome you to our practice! If this is your first time visiting the Harborside Dental Team, we ask that you complete the online Patient Information Forms prior to your arrival. This will ensure a quick check-in and be seen as soon as possible.
If you are unable to make the appointment you have scheduled with the Harborside Dental Team, please call us at least 48 hours in advance. A minimum charge will be made for failed or cancelled appointments without prior notification. We would be glad to reschedule the appointment at a more convenient time, if necessary.
